On or about December 4, 2012, at a workplace located at the intersection of Thornton Parkway and Hwy 820 West North Richland Hills, Texas; employees, who were working inside an 8 foot trench box performing pipe installation work in an 11-foot deep trench that did not completely shield the west side of the trench box. The employees were exposed to approximately (3) feet of soil, pipe installation materials and a Stihl Cut-Off Saw exposing the employees to the hazard of a cave-in and struck-by hazards.
