29 CFR 1910.178(q)(1): Any power-operated industrial truck not in safe operating condition was not taken out of service, and/or repairs were not made by authorized personnel:  a) Toyota Model 5FGC15 propane forklift, S/N 71079  A forklift used inside the warehouse was not in safe operating condition:  it had a broken tank clamp, damaged seat belt and discharged 900 parts per million (ppm) carbon monoxide in its exhaust.  Violation occurred on or about 12/6/12.   b) Toyota Model 5FGC15 propane forklift, S/N 72155   A forklift used inside the warehouse was not in safe operating condition:  it had two broken rear lights and discharged at least 2000 ppm carbon monoxide it its exhaust.  Violation occurred on or about 12/6/12.   c) Toyota Model 5FGC15 propane forklift, S/N 72687  A forklift used inside the warehouse was not in safe operating condition:  It had a damaged, inoperable seat belt and discharged at least at least 2000 ppm carbon monoxide in its exhaust.  Violation occurred on or about 12/6/12.   d)  Clark Model CGC25 propane forklift, S/N C3656-0077-9452FB   A forklift used inside the warehouse was not in safe operating condition:  It had a broken load backrest and discharged 1500 ppm carbon monoxide in its exhaust.  Violation occurred on or about 12/6/12.

29 CFR 1910.1000(a)(2): Employee(s) were exposed to an airborne concentration of carbon monoxide listed in Table Z-1 in excess of the 8 hour Time Weighted Average concentration of 50 parts per million (ppm):    a) Warehouse    Three employees were exposed to carbon monoxide at levels of 85 ppm, 257 ppm and 375 ppm as an 8 hour Time Weighted Average, which were 1.7, 4.9 and 7.5 times the OSHA Permissible Exposure Limit of 50 ppm, respectively.  Exposure periods were 345, 440 and 435 minutes, respectively.  Exposures were calculated from the percent carboxyhemoglobin in the blood and other factors such as atmospheric pressure, height, weight, activity levels and tobacco smoking.  Zero exposure was assumed for 135, 40 and 45 minutes, respectively, outside the exposure period.    Violation occurred on or about 12/4/12.
