(a) Location -   93 Marblehead Street North Reading, MA, 01864  On or about 12/04/12, employees were not protected from scaffold collapse while working on a ladder jack scaffold with the platform extending more than 12 inches over its supports.    Baystate Roofers, Inc. was previously issued a citation for a similar violation 1926.451(b)(5)(ii) for having a ladder jack scaffold platform extending over its supports more than 18 inches when the platform supports were greater than 10 feet apart. This citation was contained in OSHA Inspection Number 315871897, Citation Number 01, Item number 002A, issued on 01/24/2012, and was affirmed as a final order on 02/16/2012 with respect to a workplace located at 43 High Street, Topsfield, MA  01983. A recent inspection of the same employer reveals a violation of 1926.451(b)(5)(i) for having a ladder jack scaffold platform extending over its supports more than 12 inches when the platform supports were less than 10 feet apart.
