29 CFR 1926.304(d): Portable, power-driven circular saw(s) were not equipped with a guard above and below the base plate or shoe: a) Front of townhous. An employee was using a seven and one quarter inch circular saw to cut siding to size for installation. The blade guard of the saw had been blocked in the fully open position exposing the employee to amputation hazards.

29 CFR 1926.1053(b)(16): Portable ladders with structural defects, such as, but not limited to, broken or missing rungs, cleats, or steps, broken or split rails, corroded components, or other faulty or defective components, were not either immediately marked in a manner that readily identifies them as defective, or tagged with "Do Not Use" or similar language, and were not withdrawn from service until repaired. a) Front of townhouse. An employee working from an elevated ladder jack working platform, 17-feet 9-inches above ground level was exposed to fall hazards as one of the extension ladders supporting the working platform was damaged. The feet of the extension ladder, which provide slip-resistant bearing surfaces, were missing.
