(a) Roof, located on the south side of the Hope Assisted Living Facility: Employees were not protected from eye injuries while installing 5/8" OSB plywood sheathing with pneumatic Hitachi (Model NR83A2) Stick Framing Nail Guns as these tools were operated without safety glasses.

29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet or more above the lower levels was not protected by guardrail systems, safety net systems, personal fall arrest systems, nor was an alternative fall protection plan which meets the requirements of 29 CFR 1926.502(k) developed and implemented: (a) Roof, located on the south side of the Hope Assisted Living Facility: Three employees were exposed to fall hazards of approximately 11'5" to the ground while installing 5/8" OSB plywood sheathing on a 6/12 pitched hip style roof with no fall protection system in place. (b) Top plates, located on the north side of the Hope Assisted Living Facility: Two employees were exposed to fall hazards of approximately 10'11" to the ground while walking the top plate installing pre-fabricated roof trusses with no fall protection system in place.
