29 CFR 1926.102(a)(1): Employees were not provided with eye and face protection equipment when machines or operations present potential eye or face injury from physical, chemical, or radiation agents:    This violation occurred on November 19, 2012; at a jobsite located at 121 W. Rock Island Road, Grand Prairie, Texas 75050; six (6) employees were working on the roof of a one-story commercial building while using hand held tools to remove the old roof without the use of eye protection.

29 CFR 1926.501(b)(10): Except as otherwise provided in paragraph (b) of this section, each employee engaged in roofing activities on low-slope roofs, with unprotected sides and edges 6 feet (1.8 m) or more above lower levels were not protected from falling by guardrail systems, safety net systems, personal fall arrest systems, or a combination of warning line system and guardrail system, warning line system and safety net system, or warning line system and personal fall arrest system, or warning line system and safety monitoring system:     This violation occurred on November 19, 2012; at a jobsite located at 121 W. Rock Island Road, Grand Prairie, Texas; where at least two (2) employees were working on the roof edge without the use of fall protection, and were exposed to a potential 23 foot fall to the ground level below.
