Site and ground conditions must be adequate for safe assembly/disassembly operations and to support the equipment during assembly/disassembly (see  1926.1402 for ground condition requirements).    a.  On or about December 4, 2012, at the above addressed jobsite, an employee was operating a Terex BT-3470 Truck Mounted Crane, the truck mounted crane was erected next to an unsupported excavation wall and underground sewer systems, thereby exposing the employee to hazards associated with cranes.

On or about December 4, 2012, at the above addressed jobsite, an employee was operating a Terex BT-3470 Truck Mounted Crane, two outriggers were supported by inappropriate blocking materials (outrigger pads), such as but not limited to, scrap wood and single sheets of plywood, thereby exposing the employee to hazards associated with cranes.
