(a)  Worksite , 289 Dune Road, Westhampton Beach, NY - An  Employee  was ripping wood on  a  Makita table saw, Model # 2703, Serial #121643A 2/12.  The saw had no non-kickback fingers or spreader as required by ANSI page 9, 4.1.2 (b) Spreader and page 10, 4.1.2 (c) Non Kickback Fingers ; on or about 12/05/12.  Note:   Because abatement of this violation is already documented in the case file, the employer need not submit certification or documentation of abatement for this violation as normally required by CFR 1903.19.
