29 CFR 1910.147(c)(1):     The employer did not establish a program consisting of an energy control procedure, employee training and periodic inspections to ensure that before any employee performed any servicing or maintenance on a machine or equipment where the unexpected energing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative:    A written lockout / tagout program had not been developed and implemented to inform employee(s) of the struck-by hazards associated with performing maintenance and preventive maintenance on hard wired equipment such as Heidel Berg QMDI, Heidel Berg GTDI, ABDICK machines, and etc.

29 CFR 1910.303(b)(2):     Listed or labeled electrical equipment was not used or installed in accordance with instructions included in the listing or labeling:    (a)  Employee(s) plugging electrical equipment into four plex metallic receptacle boxes that were attached to the end of ceiling drop down power cords in the production department were exposed to electrical hazards due to metallic receptacle boxes not being installed to a structure according to the listing / labeling instructions.    (b)  Employee(s) plugging electrical equipment into four plex metallic receptacle boxes that were attached to the end of ceiling drop down power cords in the production department were laying on the production floor, thus exposing employees to electrical hazards due to metallic receptacle boxes not being installed to a structure according to the listing / labeling instructions.    (c)  Employee(s) plugging electrical equipment into a metallic receptacle box that was wired to a junction box that was suspended in the air by a drop down power cord were exposed to electrical hazards due to the junction box not being installed to a structure according to the listing / labeling instructions.
