29 CFR 1910.303(g)(2)(i): Except as elsewhere required or permitted by Subpart S of Part 1910, live parts of electric equipment operating at 50 volts or more were not guarded against accidental contact by use of approved cabinets or other forms of approved enclosures: a.) Rigging Shop, Restroom. On or about December 4, 2012, the on/off switch for the overhead light was missing a cover plate thereby exposing employees to inadvertent contact with live electrical parts.

Main Shop, employee work bench near heating unit, Power Sentry power strip, Model 100173, 120V, 15 amp, 60 Hz. On or about December 4, 2012, a six outlet power strip was not used to its listed or labeled purpose in that a portable 12 inch oscillating table fan (appliance) was plugged into the unit.
