29 CFR 1926.501(b)(10): Each employee engaged in roofing activities on low-slope roofs with unprotected sides and edges 6 feet or more above lower levels, was not protected from falling by guardrail systems, safety net systems, personal fall arrest systems, or a combination of warning line system and fuardrail system, warning line system and safety net system, or warning line system and personal fall arrest system, or warning line system and safety monitoring system.  Or, on roofs 50-feet (15.25 m) or less in width, each employee was not protected by use of a monitoring system:    a) Roof of convenience store. The employee, who was not wearing a personal fall arrest system, while leaning over the unprotected edge of the roof to receive sheets of roof sheathing, was exposed to a fall hazard of approximately 15-feet.

29 CFR 1926.1053(b)(4): Ladders shall be used only for the purpose for which they were designed:    a) An employee, who was passing material to another employee on the roof, was observed working from a self-supporting portable ladder that was leaning against the front exterior wall of the building. The ladder, which had not been opened and locked into position, was being used for a purpose other than what it was designed for.

29 CFR 1926.1060(a): The employer did not provide a training program for each employee using ladders and stairways, as necessary, which would enable each employee to recognize hazards related to ladders and stairways and train each employee in the procedures to be followed to minimize these hazards:    a) Front of convenience store located at 2561 Windy Hill Road in Marietta, Georgia. An employee who was working from a self-supporting portable ladder that was leaning against the front exterior wall of the building. The ladder, which had not been opened and locked into position, was being used for a purpose other than what it was designed for. The employer had not provided ladder safety training to the employee working from the ladder.
