29 CFR 1926.652(a)(1): Each employee in an excavation was not protected from cave-ins by an adequate protective system designed in accordance with 29 CFR 1926.652(c). Jobsite: On or about 12/06/2012, an employee working in an excavation was not adequately protected from cave-ins in that the trench box was placed approximately 15 inches below the surface of the excavation.
