29 CFR 1926.95(a): Personal protective equipment, including personal protective equipment for eyes, face, head, and extremities, protective clothing, respiratory devices, and protective shields and barriers, was not provided, used, or  maintained in a sanitary and reliable condition wherever it is necessary by reason of hazards of processes or environment:         a)  Jobsite, intersection of Duncan Avenue and Route 8, Allison Park, Pennsylvania, 15101:  Observed on or about December 6, 2012, employees working in an excavation which measured approximately 12.4 foot deep were not wearing protective helmets.  The employees were exposed to struck-by hazards from rocks, tools, and materials located on the edges of the excavation.

The employees were exposed to engulfment and struck-by hazards while attempting to install a 24 inch pipe beneath Duncan Avenue.

The employees were exposed to engulfment and struck-by hazards while attempting to install a 24 inch pipe beneath Duncan Avenue.
