29 CFR 1910.67(c)(2)(v): A body belt was not worn and a lanyard attached to the boom or basket when working from an aerial lift: a) At Entrance to Eastern Shore Mall Spanish Fort, Alabama: On or about December 6, 2012 and at times prior the employer failed to ensure employees working from an elevated sky hoist bucket were wearing a harness and tied in to prevent the employees from falls of 21.5 feet to the ground below. b) At Entrance to Eastern Shore Mall Spanish Fort, Alabama: On or about December 6, 2012 and at times prior the employer failed to ensure employees working from an elevated Elliot L60 hoist bucket were wearing a harness and tied in to prevent the employees from falls of 16 feet to the ground below.
