29 CFR 1926.651(k)(1): Daily inspections of excavations, the adjacent areas, and protective systems were not made by a competent person for evidence of a situation that could have resulted in possible cave-ins, indications of failure of protective systems: a) Work Site at Kuehne Chemical, 86 Hackensack Ave, Kearny, NJ: Employers designated competent person for the excavation did not conduct an inspection of it prior to assigning sub-contractor employees to access it. Excavation had visible deficiencies, such as but not limited to, no protective cave-in system with a depth of approximately 8ft. Violation occurred on or about 12/06/2012
