On or about December 6, 2012 at the above addressed jobsite, employees were exposed to fall hazards by the presence of a floor opening. PLEASE NOTE: CERTIFICATION OF ABATEMENT IS REQUIRED FOR THIS VIOLATION.

On or about December 06, 2012, at the above addressed jobsite, the toprail of the platform level did not have a vertical height of 42 inches thereby exposing employees to the hazrds associated with falls.
