29 CFR 1910.1200(e)(1):  The employer did not develop, implement, and/or maintain at the workplace a written hazard communication program which describes how the criteria specified in 29 CFR 1910.1200(f), (g), and (h) will be met:    a)  Embalming Room - Employees were required to work with chemicals, such as but not limited to: Care 25 (containing formaldehyde and methanol), Triton 28 (containing formaldehyde and methanol), and Cavicide (containing formaldehyde, methanol, and ortho-phenylphenol).  A written hazard communication program had not been established or implemented to discuss how the employer plans to address the labeling of chemicals, safety data sheets, and employee training.  Employees could develop cancer or suffer from severe irritation of the throat, nose, and lungs.

The company did not create or maintain a chemical inventory list for all hazardous chemicals the company provided.  Employees could develop cancer or suffer from severe irritation of the throat, nose, and lungs.
