29 CFR 1926.404(b)(1)(ii): Receptacles on a three-wire, dual voltage 120v/240v portable or vehicle-mounted generator rated at 5kW or more, where the circuit conductors of the generator are not insulated from the generator frame and all other grounded surfaces, were not be protected with ground-fault circuit interrupters. (a) Institute of Science and technology Training Center, Bellefonte Pennsylvania - Employees used a Honda EM5000S dual voltage portable generator without a ground fault circuit interrupter to power a Dewalt DW367 circular saw and a Dewalt D25103 Hammer Drill on or about December 6, 2012. Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.

29 CFR 1926.405(g)(2)(iv): Flexible cords were not connected to devices and fittings so that strain relief is provided to prevent pull from being directly transmitted to joints or terminal screws: (a) Institute of Science and technology Training Center, Bellefonte Pennsylvania - Employees used an extension cord with wires pulled from the strain relief of the receptacle to power a Dewalt DW367 circular saw on or about December 6, 2012. Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.
