29 CFR 1926.150(c)(1)(i): A fire extinguisher, rated not less than 2A, was not provided for each 3,000 square feet of the protected building area, or major fraction thereof: On or about December 6, 2012 at the construction site of an apartment complex fire extinguishers were not provided. This created a fire hazard to framer at the construction site.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b): On or about December 6, 2012 workers did not have fall protection at the site. Workers stand on a wood beam and saw off one end at 9 feet of elevation. This is a fall from elevation hazard to framers.
