29 CFR 1926.300(b)(1): A power operated tool designed to accommodate guards was being operated when the guard was not installed. On or about 11/30/2012, employees were using a 4 & 1/2" Dewalt grinder, model # D28482, at 1601 South Jefferson, St. Louis, MO 63104.

29 CFR 1926.405(g)(2)(IV) Flexible cords were found without proper strain relief. On or about 11/30/2012, employees were using working at 1601 South Jefferson, St. Louis, MO 63104. They were using a Sky Jack lift which had a faulty connection for the battery charger. Specifically, the cord was not pushed far enough into the electrical connection which wiring. Thus, the electrical wires were exposed where the insulation stopped and the connection began.
