29 CFR 1910.147(c)(5)(i): Locks, tags, chains, wedges, key blocks, adapter pins, self-locking fasteners, or other hardware shall be provided by the employer for isolating, securing or blocking of machines or equipment from energy sources: (a) Employees were not provided with locks and tags to attach to energy isolating devices prior to replacing the abrasive wheels on the centerless grinders and prior to replacing the blades on the band saws. In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification. This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence, or written records.

29 CFR 1910.147(c)(7)(i): The employer shall provide training to ensure that the purpose and function of the energy control program are understood by employees and that the knowledge and skills required for the safe application, usage, and removal of the energy controls are acquired by employees: (a) Employees changing the blade on the DoAll Horizontal Band Saw and the DoAll Vertical Band Saw, were not adequately trained on the energy control program, in that they did not isolate the electrical energy source and then attach a lock to the energy isolating device. (b) Employees changing the grinding wheels on the centerless grinders were not adequately trained on the energy control program, in that they did not isolate the electrical energy source and then attach a lock to the energy isolating device. 29 CFR 1910.212(a)(1): Types of guarding. One or more methods of machine guarding shall be provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks. Examples of guarding methods are-barrier guards, two-hand tripping devices, electronic safety devices, etc.: (a) The guards on the Brown and Sharp 55-100 CNC Lathe did not completely enclose the machine, thereby exposing the operators to potential injuries should the parts or tooling fly out. (b) The guards on the Tornos Swiss Turn (unnumbered) did not completely enclose the machine, thereby exposing the operators to potential injuries should the parts or tooling fly out. (c) The guards on the Tsugami Swiss Turning Centers #61-027 and 61-029 were not interlocked with the machine control system and did not completely enclose the machines, thereby exposing the operators to potential injuries should the parts or tooling fly out. (d) Employees operating the centerless grinders were exposed to the turning wheels when they held the parts between the wheels and when they reached under the wheels to remove parts. (e) Employees operating the Enco Shear were exposed to a partially guarded blade, in that there were 20 gaps in the guard measuring 1-1/8 by 1-1/4 inches. (f) Employees operating the Delta Multi Spindle Drill Press and the Edlung Multi Spindle Drill Press were exposed to the unguarded rotating tools. (g) Employees operating the DoAll Vertical Band Saw, were exposed to 2 inches of unguarded blade, where the blade ran through the guides. In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification. This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence, or written records.
