Brandon Stand Rd.: On December 3, 2012 and at times prior to; the employer provided a tool that was not maintained in a safe condition in that the Dewalt grinder model D28114 was used without a handle, exposing employees to cuts and amputation hazards while cutting a line in brick to insert flashing.

29 CFR 1926.501(b)(11): "Steep roofs." Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels shall be protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems. (a) At 4205 Brandon Stand Rd., Taylor, AL: On December 3, and at times prior to; the employer failed to ensure that employees were protected from falls exposing the employees to falls of approximately 11.5 feet while installing caps on a 12:10 pitched, steel roof of a church.
