On or about December 6, 2012, and times prior thereto, the employer, whose employees were exposed to fire hazards, did not provide an exit sign above the doors located in: a) in the Air-Lock room, north west of the facility, where wood materials are stored. b) in the End Fold Area, north east of the facility, where flammable lockers or stored.

29 CFR 1910.305(b)(2): Pull boxes, junction boxes, and fittings were not provided with covers. On or about December 6, 2012, and times prior thereto, in the Lower Lock Former Kit Roller area, the employer did not ensure a cover plate was placed over the junction box, that connects to the Cerrado motor of the Nip Roller. The employees were exposed to electrical hazards.

29 CFR 1910.305(g)(1)(iii): Flexible cords and cables were used for purposes prohibited by subparagraphs (A) through (E) of this paragraph. On or about December 6, 2012, and times prior thereto, at the KA Counter, the employer exposed employees to electrical fire hazards. The employer: a) spliced an extension cord into a motor to energize a fan, and b) utilized an extension cord as permanent wiring to energize the visual read out/KA counter.
