29 CFR 1926.501(b)(11) "Steep roofs." Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels shall be protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems. (a) East side of building. An employee was observed working on a steep pitch roof, approximately 22 feet above the ground below, without fall protection, on or about 12/7/12. Note: In addition to abatement certification, the employer is required to submit abatement documentation for this item in accordance with 29 CFR 1903.19.

29 CFR 1926.1053(b)(1) Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed: (a) West side of building. Employee was exposed to fall hazards in that the portable ladder used to access the roof extended 18 inches beyond the landing, on or about 12/7/12. Note: The employer is required to submit abatement certification for this item in accordance with 29 CFR 1903.19.
