29 CFR 1910.145(f)(3): Tags were not used as a means to prevent accidental injury or illness to employees who were exposed to hazardous or potentially hazardous conditions, equipment or operations which were out of the ordinary, unexpected or not readily apparent. (a) In the Shop: On December 6, 2012 and at times prior to; the employer failed to use tags (not for use) on equipment such as, but not limited to miter saws and table saws that were not guarded or un-safe to use in the cabinet shop environment, exposing employees to amputation hazards.
