29 CFR 1926.451(g)(4)(ii): The top edge height on all suspended scaffolds where both a guardrail and a personal fall arrest system were not between 36 inches (0.9 m) and 45 inches (1.2 m) above the platform surface:Note:  For supported scaffolds manufactured and placed in service before January 1, 2000, and on all suspended scaffolds where both a guardrail and a personal fall arrest system are required, the top edge height should be between 36 inches (0.9 m) and 45 inches.      a) On or about 12-07-2012 in front of building 1617, a complete guardrail system was not provided for the employee applying stucco to the building from a scaffold exposing the employee to a 13 ft. fall hazard.
