29 CFR 1910.146(f): The entry permit that documents compliance with this section and authorizes entry did not identify the required information listed in (f)(1)-(f)(15):    On or about December 7, 2012, the permit for entry into the baby food mixer did not include the following:    a.  The name of the space and the purpose for entering the baby food mixer.-      b.  The hazards to possibly be encountered including lack of oxygen and presence of hexavalent chromium.    c.  The equipment needed to be worn to ensure rescue including, but not limited to, a harness worn by the entrant.    d.  The type of lighting required inside the baby food mixer to perform the required tasks.    e.  The personal protective equipment required during entry when welding, polishing, or grinding is done.    *Abatement certificaiton is required per 1903.19(c).

29 CFR 1910.146(g)(1):     The employer did not provide training so that all employees whose work is regulated by this section on permit required confined space entry acquired the understanding, knowledge, and skills necessary for the safe performance of the duties assigned under this section:    On or about December 7, 2013, employee(s) performing duties associated with the permit required confined space entry of the baby food mixer had not acquired the understanding, knowledge and skill needed:    a.  Employee(s) were not knowledgeable about the need to monitor the oxygen content at all levels of the vessel.    b.  Employee(s) were not knowledgeable about the duty to review all entry permits for the Baby Food Mixer to determine if confined space entry procedures were correctly followed.    c.  Employee(s) in charge of the confined space program, were unable to identify the Baby Food Mixer as a permit required confined space.    d.  Employee(s) did not know the hazards that may have been faced during entry, including information on the signs and symptoms of exposure.    *Abatement certification is required per 29 CFR 1903.19(c).

29 CFR 1910.146(j): The employer did not ensure that the entry supervisor for permit required confined space entry was trained to perform the duties required by (j)(1)-(6) of this section (29 CFR 1910.146):  On or about December 7, 2013, the employer did not ensure that the entry supervisor for the permit required confined space entry into the baby food mixer:  a.  Knew the hazards faced during entry, including, but not limited to, oxygen deficiency and exposure to hexavalent chromium when welding, polishing and grinding stainless steel.  b.  Knew all required entries on the permit and filled them out appropriately, including, but not limited to, oxygen level readings, the identity of the confined space, the purpose of entry, the time entered and duration of the permit, the equipment needed for rescue, the type of lighting required, and the appropriate personal protective equipment needed during entry.  c. Was knowledgeable of rescue procedures and equipment required such as a harness for retrieval.  *Abatement certification is required per 29 CFR 1903.19(c).

29 CFR 1910.178(q)(6): Powered industrial trucks were altered so that the relative positions of the various parts were different from what they were when originally received from the manufacturer, by the addition of extra parts not provided by the manufacturer, or by the elimination of any parts without the manufacturer's approval:    The tynes of the Hyster #507 fork lift in Plant #3 had large holes drilled in them for affixing loads without the manufacturer having given permission to operate the lift with compromised tynes.     *Abatement documentation is required per 29 CFR 1903.19(d).
