29 CFR 1910.151(b): There was neither an infirmary, clinic, or hospital used for the treatment of all injured employees in near proximity to the workplace nor a person or persons adequately trained to render first aid: Well site: On or about December 3, 2012, employees were rigging up (erecting) a pole type service rig to repair an oil/gas well. The employer had not provideded first aid training to employees and there was no other medical assistance close to the work site. Employees were exposed to hazards from delayed medical attention in the event of an accident.

29 CFR 1910.333(c)(3)(i)(B): When an unqualified person was working on the ground in the vicinity of overhead lines, the person brought a conductive objected closer to unguarded, enerigized overhead lines that the distance given in Paragraph (c)(3)(i)(A) of this section: Well site: On or about December 3, 2012, employees were rigging up (erecting) a pole type service rig to repair an oil/gas well. An employee staking out the rig's rear wire rope guy line contacted an overhead electrical line. This exposed the employees to the hazard of electrical shock.
