29 CFR 1910.1030(f)(2)(i):  Hepatitis B vaccination was not made available after the employee has received the training required in paragraph (g)(2)(vii)(I) and within 10 working days of initial assignment to employees who have occupational exposure to blood or other potentially infectious materials:    a) On or about December 4, 2012 and times prior, employees were exposed to illnesses in that, the hepatitis B vaccination was not made available prior to placing these employees into the work place.     Sun Health Care Group dba Pueblo Care & Rehabilitation Center was previously cited for a violation of this occupational safety and health standard, which was contained in OSHA Inspection Number 110485968, Citation Number 1, Item Number 2a, and issued on 08/06/2008, with respect to a workplace located at 2611 Jones Avenue, Pueblo, CO 81004. This citation became final order on October 21, 2008.    Abatement Note:  Abatement certification and documentation is required for this item (see enclosed "Certification of Corrective Action Worksheet").

Self-luminous or electroluminescent signs that have a minimum luminance surface value of at least .06 foot lamberts (0.21 cd/m2) are permitted:    a) On or about December 4, 2012 and times prior, the exit sign located in the therapy room area was not illuminated as required.
