OSH ACT of 1970 Section (5)(a)(1): The employer failed to furnish to each of his employees employment and a place of employment which was free from recognized hazards that were causing or are likely to cause death or serious physical harm, in that, two employees were injured when a 25 metric ton overhead bridge crane struck the basket of an aerial lift, subsequently tipping the lift over backwards: a) The F line of the production area of the facility. While positioning an aerial lift to conduct maintenance on components of the fire suppression system two employees were injured when an overhead bridge crane struck the basket of an aerial lift subsequently tipping the lift over backwards. The basket of the aerial lift was elevated, between the tracks of the crane, approximately 45-feet when it was hit and knocked over by the 25 metric ton overhead bridge crane.

29 CFR 1910.147(a)(3)(i): The employer did not utilize procedures for affixing appropriate lockout devices or tagout devices to energy isolating devices, and to otherwise disable machines or equipment to prevent unexpected energization, start up or release of stored energy in order to prevent injury to employees: a) The F line of the production area of the facility. While positioning an aerial lift to conduct maintenance on components of the fire suppression system two employees were injured when an overhead bridge crane struck the basket of the aerial lift subsequently tipping the lift over backwards. The basket of the aerial lift was elevated approximately 45-feet when it was hit and knocked over by the 25 metric ton overhead bridge crane. The crane had not been lockouted to prevent energization, movement, while employees were working from the elevated aerial lift basket positioned between the tracks of the crane.

The employer failed to ensure a 25 metric ton overhead bridge crane was locked out prior to maintenance personnel elevating an aerial lift between the tracks of the cranes runway to resolve problems with a duct detector for the air handling unit and a ground fault on the alarm horn strobe lights. The Sewon maintenance technician had not received any specific lockout/tagout training regarding the methods and means necessary to lockout the overhead bridge crane.

The employer failed to ensure a 25 metric ton overhead bridge crane was locked out prior to maintenance personnel elevating an aerial lift between the tracks of the cranes runway to resolve problems with a duct detector for the air handling unit and a ground fault on the alarm horn strobe lights. The subcontracted maintenance technician from AFA Protective Services had not received any instruction regarding the purpose for and the use of the energy control procedures to lockout the overhead bridge crane.
