29 CFR 1926.451(b)(1): Each platform on all working levels of scaffolds was not fully planked or decked between the front uprights and the guardrail supports as specified in paragraphs 1926.451(b)(1)(i)-(ii): a) On or about December 10, 2012 and at times prior thereto at the job site; employee was exposed to a 15' fall hazard while working from the top working level of the scaffold with only one of two hook planks installed. Decked work platform width was approximately 24", no additional guardrails and guardrail supports were installed.
