The railing shall be provided with a toeboard wherever, beneath the open sides,    a.) Avenger Warehouse: On or about 12/06/12, a guardrail system was not provided along the open edge of a storage area that was 11' above the warehouse floor while employees stowed material thereby exposing employees to a fall hazard.

One or more methods of machine guarding shall be provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks. 29 CFR 1910.304(b)(3)(ii)(A):     All 125-volt, single-phase, 15-, 20-, and 30-ampere receptacle outlets that are not part of the permanent wiring of the building or structure and that are in use by personnel shall have ground-fault circuit-interrupter protection for personnel.  Note 1 to paragraph (b)(3)(ii)(A) of this section: A cord connector on an extension cord set is considered to be a receptacle outlet if the cord set is used for temporary electric power.  Note 2 to paragraph (b)(3)(ii)(A) of this section: Cord sets and devices incorporating the required ground-fault circuit-interrupter that are connected to the receptacle closest to the source of power are acceptable forms of protection.    a.) Fabrication Shop: On or about 12/07/12, the 115 volts receptacle outlet on the Miller Synrowave 300 portable welding unit was not provided with GFCI protection while portable electrical fans were plugged into the receptacle.

A clear space of not less than 3 feet on all sides shall be kept free from storage or combustible construction.    a.) Fabrication Shop: On or about 12/06/12, a clear space along the left side of the large spray booth was not more than 3' in that a large metal storage rack was erected approximately 1' away thereby restricting access for cleaning.

29 CFR 1915.112(c)(2):     All sling chains, including end fastenings, shall be given a visual inspection before being used on the job. A thorough inspection of all chains in use shall be made every 3 months. Each chain shall bear an indication of the month in which it was thoroughly inspected. The thorough inspection shall include inspection for wear, defective welds, deformation and increase in length or stretch.    a.) Fabrication Shop: On or about 12/07/12, a thorough inspection of the 72" chain sling was not conducted every three months.
