29 CFR 1926.21(b)(6)(i):     Employees required to enter into confined or enclosed spaces were not instructed as to the nature of the hazards involved, the necessary precautions to be taken, and in the use of protective and emergency equipment required:    a)  The employer did not ensure that employees required to confined or enclosed spaces were instructed as to the nature of the hazards involved, the necessary precautions to be taken, and in the use of protective and emergency equipment required.  This violation occurred on or about December 11, 2012, when employees were required to enter the underground chamber of a sewerage lift plant to install new valves and were not trained in the nature of hydrogen sulfide gas, precautions to be taken, or the use of protective and emergency equipment required.    b)  The employer did not ensure that employees required to confined or enclosed spaces were instructed as to the nature of the hazards involved, the necessary precautions to be taken, and in the use of protective and emergency equipment required.  This violation occurred on or about December 11, 2012, when employees were required to enter the underground chamber of a sewerage lift plant to install new valves and were not trained in the nature of carbon monoxide gas, precautions to be taken, or the use of protective and emergency equipment required.
