29 CFR 1926.403(i)(2)(i): Live parts of electric equipment operating at 50 volts or more were not guarded against accidental contact: a) On or about October 25, 2012 and at times prior, Calvin Turner Roofing as the controlling contractor did not ensure that workers were protected from electrical hazards. Employees of a subcontractor did remove and install metal sheet roofing on a shop where the power lines came within two feet of the peak. The power lines ranged from 1280 - 3600 kv.

29 CFR 1926.501(b)(10): Each employee engaged in roofing activities on low-slope roofs, with unprotected sides and edges 6 feet (1.8 m) or more above lower levels, was not protected from falling by guardrail systems, safety net systems, personal fall arrest system, or other approved system: a) On or about October 25, 2012 and at times prior, Calvin Turner Roofing as the controlling contractor, did not ensure that workers were protected from fall hazards. The roofing crew did workl, stocking material and removing old metal, on the lower level of the structure. They did not use fall protection.
