(b) Facility, Number 35 Slitter MCC Gear, Switch 9: On or about October 3, 2012, employees were exposed to live electrical parts of 480 Volts AC 3 Phase when a switch broke and was missing requiring the door to be opened to achieve energy isolation.      (c) Facility, Number 35 Slitter MCC Gear, Switch 5: On or about October 3, 2012, employees were exposed to live electrical parts of 480 Volts AC 3 Phase when a switch broke and was missing requiring the door to be opened to achieve energy isolation.      (d) Facility, Doghouse 3-5, Number 4 Tunnel Furnace Zone 4 and 5: On or before December 10, 2012, the employer did not replace a broken switch to isolate zones 4 and 5.    (e) Facility, Crane Disconnect, Hot Rail Columns 51 to 66: On or before December 10, 2012, employees were exposed to fire and/or arcing part hazards when a disconnect switch was not fully engaged.    (f) Facility, Number 3 Tunnel Furnace, Zone 1: On or before December 10, 2012, the employer did not replace a broken switch to isolate zone 1.     (g) Facility, Number 3 Tunnel Furnace, Zone 2: On or before December 10, 2012, the employer did not replace a broken switch to isolate zone 2.    (h) Facility, Number 3 Tunnel Furnace, Zone 4 and : On or before December 10, 2012, the employer did not replace a broken switch to isolate zone 4 and 5.    (i) Facility, Number 3 Tunnel Furnace, Zone 6: On or before December 10, 2012, the employer did not replace a broken switch to isolate zone 6.    (j) Facility, Number 3 Tunnel Furnace, Zone 7: On or before December 10, 2012, the employer did not replace a broken switch to isolate zone 7. (b) Facility, Doghouse 3-5, Top Box: On or about November 19, 2012, employees were exposed to live electrical parts powered at 120 Volts AC when a cover was missing on a Femco box.     (c) Facility, Doghouse 6-9, Bottom Box: On or about November 19, 2012, employees were exposed to live electrical parts powered at 120 Volts AC when a cover was missing on a Femco box.     (d) Facility, Doghouse 6-9, Top Box: On or about November 19, 2012, employees were exposed to live electrical parts powered at 120 Volts AC when a cover was missing on a Femco box.     (e) Facility, Doghouse 1-3, Top Box: On or about November 19, 2012, employees were exposed to live electrical parts powered at 120 Volts AC when a cover was missing on a Femco box.     (f) Facility, Doghouse 1-3, Raceway: On or about November 19, 2012, a cover was not secured along the bottom of the pull box creating a gap of approximately one inch.     (g) Facility, Solenoid Cabinet: On or about November 19, 2012, employees were exposed to live electrical parts powered at 120 Volts AC when a cover was hinged in the open position.     (h) Facility, Number 35 MCC Gear, Number Five Welder AC Disconnect: On or about October 3, 2012, employees were exposed to live electrical parts of 480 Volts AC 3 Phase when a switch was missing creating a hole in the cabinet door.
