29 CFR 1926.102(a)(1): Eye and face protective equipment were not used when machines or operations presented potential eye or face injury :      JOB SITE: 6217 Rhodes Avenue, Saint Louis, MO 63109, at this work site, two workers were observed using pneumatic nail guns, actively installing new roof shingles, without any form of eye or face protection.  The workers were exposed to struck-by hazard.

JOB SITE: 6217 Rhodes Avenue, Saint Louis, MO 63109, at this work site, a worker was observed on top of the residential structure, a 4:12 pitch roof, approximately 16 feet 7 inches eave to ground level, actively installing new roofing shingles, wearing used a body belt as Personal Fall Arrest System and was not attached in the center of wearer's back.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measures:       JOB SITE: 6217 Rhodes Avenue, Saint Louis, MO 63109, at this work site, workers were observed on top of a 4:12 pitch roof, approximately 16 feet 3 inches eave to ground level, actively removing old roof shingles, with one worker wearing personal fall arrest systems, but was not tied down with lanyard to an anchor point and second worker wearing a body belt as personal fall arrest system nor were employees provided with an alternative fall protection.  The workers were exposed to fall hazard.    Frederic Roofing & Sheet Metal Company, Inc., was previously cited for a violation of this occupational safety and health standard or its equivalent standard of 29 CFR 1926.501(b)(10), which was contained in OSHA inspection number 278994, citation number 1, item 1, issued on 05/29/2012 and was affirmed as a final order date of 06/21/2012, with respect to a workplace located on 7500 Morganford Road, Saint Louis, MO 63116.    Frederic Roofing & Sheet Metal Company, Inc., was previously cited for a violation of this occupational safety and health standard or its equivalent standard of 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 314016593, citation number 1, item 2, issued on 12/04/2009 and was affirmed as a final order date of 12/31/2009, with respect to a workplace located on 6200 S. Lindbergh, Saint Louis, MO 63123.
