29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper level landing surface and the ladders length did not allow the ladder side rails to extend at least 3 feet (0.9 m) above the upper landing surface being accessed, the ladder was not secured at its top to a rigied support that will not deflect, and/or a grasping device was not provided to assist emmployees in mounting and dismounting the ladder. Location: Intersection of Beach Street & East Avenue, West Haven, CT 06516 The 20 foot orange fiberglass extension ladder placed between the bottom of the excavation and the top of the 72 inch diameter storm sewer pipe did not extend at least three feet above the top of the pipe and not secured to prevent displacement.
