29 CFR 1926.100(a): Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets:  Jobsite: Site.  Workers were exposed to serious head injury by being struck by falling objects such as tools and materials while walking/working below other workers that were working on the roof where head protection was not worn.

29 CFR 1926.501(b)(10): Each employee engaged in roofing activities on low-slope roofs, with unprotected sides and edges 6 feet (1.8 m) or more above lower levels, was not protected from falling by guardrail systems, safety net systems, or personal fall arrest:  Jobsite: Roof:  Employees were exposed to up to approximately 25 feet falls while performing roofing work where a fall protection system had not been provided.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladders length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:  Jobsite: Roof.  Employees were exposed to falls of up to 15 feet while accessing the upper roof where a step ladder was used and the side rails were below the upper landing.
