29 CFR 1910.1030(c)(1)(iv): The Exposure Control Plan was not reviewed and updated when necessary to reflect new or modified tasks and procedures which affect occupational exposure and to reflect new or revised employee positions with occupational exposure:    a)  On or about December 5, 2012, and at times prior, the employer did not conduct or document annual reviews of the facility's exposure control plan. 29 CFR 1910.1030(c)(1)(v):  The employer, who is required to establish an Exposure Control Plan, did not solicit input from non-managerial employees responsible for direct patient care who are potentially exposed to injuries from contaminated sharps in the identification, evaluation and selection of effective engineering and work practice controls and did not document the solicitation in the Exposure Control plan:    a)  On or about December 5, 2012, and at times prior, the employer did not solicit or document input from non-managerial staff for the facility's exposure control plan.      Abatement Note:  Abatement certification is required for this item (see enclosed "Certification of Corrective Action Worksheet").

29 CFR 1910.1200(f)(5)(i): The employer did not ensure that each container of hazardous chemicals in the workplace was labeled, tagged or marked with the identity of the hazardous chemical(s) contained therein:    a)  On or about December 5, 2012, and at times prior, spray bottles of 10% bleach solution in a patient room and in the dirty utilities room were not labeled with the contents of the bottle.
