29 CFR 1910.178(l)(4)(iii): An evaluation of each powered industrial truck operator's performance was not being conducted at least once every three years:    The employer failed to evaluate each authorized forklift operator every three years. Employees were operating powered industrial vehicles to load and unload materials received and to transport material to the different workstations.     Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).

29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:     Employees are exposed to cuts lacerations, fractures and amputations while operating the following equipment due to ingoing nip points and rotating parts:       a)    3925 S&S Flexographic Folder Gluer. -Missing cover on printing side.   b)    3926 Martin 616 Miniline Flexographic Folder Gluer. Missing nip point guard on small conveyor near slitter. Missing side panels near stacker.                    Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).
