29 CFR 1910.147(c)(4)(ii):     The energy control procedures did not clearly and specifically outline the scope, purpose, authorization, rules, and techniques to be utilized for the control of hazardous energy, including, but not limited to Items A-D of this section:    a)  At the establishment, on or about 12/12/12:  Where employees perform servicing and/or maintenance on various machinery, including, but not limited to printing presses, folding machines, baler, guillotine cutter, etc., the employer had not established machine specific energy control procedures as outlined in Items A-D of this section, to prevent the unexpected energizing, start up or release of stored energy of the equipment.    Abatement certification must be submitted for this item.

29 CFR 1910.304(f)(1)(iv):     Overcurrent devices for circuits rated 600 volts, nominal, or less, were not readily accessible to each employee or authorized building management personnel:    a)  Pre Press Room, on or about 12/12/12:  A circuit breaker panel and disconnect switch next to the film processor were blocked by barrels of fixer.    b)  Electrical Room off Press Room, on or about 12/12/12:  Four circuit breaker panels were blocked by cardboard boxes, metal container, etc.    c)  Baler Room, on or about 12/12/12:  A circuit breaker panel and two disconnect switches were blocked by pallets of material.    Abatement certification must be submitted for these items.
