29 CFR 1926.403(b)(2): Listed, labeled, or certified equipment was not installed and used in accordance with instructions included in the listing, labeling, or certification: a) On or about December 7, 2012; second floor; employees were using a power strip/tap to provide power to portable equipment such as but not limited to saws; either directly or through portable extension cords. The power strips/taps are not designed for outdoor or construction sites. The use of the equipment in this environment exposes the employees to an electrical shock. No abatement certification or documentation required.

29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels was not protected by guardrail systems, safety net systems, personal fall arrest systems, or an alternative fall protection measure under another provision of paragraph 1926.501 (b), nor did the employer demonstrate that it is infeasible or creates a greater hazard to use these systems and develop and implement a fall protection plan which meets the requirements of paragraph (k) of section 1926.502: a) On or about December 7, 2012; roof area; employees were working on the top plate and beams of a residential home without a personal fall arrest system in place to protect them from falling 40 feet to ground level. No abatement certification or documentation required.

29 CFR 1926.1053(b)(1): Where (a) portable ladder(s) was/were used for access to an upper level landing surface and the ladder���s length did not allow the ladder side rails to extend at least 3 feet (0.9 m) above the upper landing surface being accessed, the ladder was not secured at its top to a rigied support that will not deflect, and/or a grasping device was not provided to assist emmployees in mounting and dismounting the ladder. a) On or about December 7, 2012; second floor; employees were accessing the beams and top plate area by using step ladders that were two feet below the landing/platform. No abatement certification or documentation required.
