29 CFR 1926.1053(b)(10): The top of a non-self-supporting ladder shall be placed with the two rails supported equally unless it is equipped with a single support attachment. a) McDonald's rooftop, north side of the hotel: Employees used a 16-foot long extension ladder to access suspended scaffolds from the rooftop. The ladder is non-self-supporting, and the top of the ladder were not placed with two rails supported equally. The ladder flipped sideways clockwise when an employee was climbing it, and the employee fell approximately 10 feet from the ladder to the rooftop. On or about 12/13/12. NOTE: THE EMPLOYER IS REQUIRED TO SUBMIT CERTIFICATION OF ABATEMENT FOR THIS ITEM. FAILURE TO COMPLY MAY RESULT IN AN ADDITIONAL PENALTY OF $1,000 AS PER 29 CFR 1903.19.
