29 CFR 1910.1200(h)(1): Employees were not provided effective information and training on hazardous chemicals in their work area at the time of their initial assignment and whenever a new hazard that the employees had not been previously trained about was introduced into their work area: On or about October 30, 2012 and times prior thereto, employees of Newoods, Inc. DBA ABC Block Company manufactured blocks and performed block maintenance in the block manufacturing shop and the maintenance shop located at 6902 Brodie Lane in Little Rock, Arkansas. The sand used by employees contained hazardous ingredients such as but not limited to silica. The employer had not trained employees on the hazards associated with sand containing silica and chemicals such as but not limited to oils and lubricants used by employees in their work areas.

29 CFR 1910.134(k)(6): The employer did not provide the basic advisory information on respirators, as presented in Appendix D of 29 CFR 1910.134, in written or oral format to employees who wear respirators when such use was not required by the employer On or about October 30, 2012 and times prior thereto, employees of Newoods, Inc. DBA ABC Block Company performed manufacturing of blocks with the use of sand in the block manufacturing shop located at 6902 Brodie Lane in Little Rock, Arkansas. The employer had provided employees with 3M N95 respirators (model 8210) to use on a voluntary basis. The employer had not provided the basic advisory information contained in Appendix D of 29 CFR 1910.134 to employees for the respirators worn.
