29 CFR 1926.416(a)(1): Employees were permitted to work in proximity to electric power circuits and were not protected against electric shock by de-energizing and grounding the circuits or effectively guarding the circuits by insulation or other means: a) At the residences on George and Edwin Streets in Brighton IL, employees were exposed to electrical hazards when employees used 2 by 4s wood members to lift up and prop the covered but non insulated 220 volt electrical distribution lines supplying power to the individual homes.

29 CFR 1926.600(a)(6): All equipment covered by this subpart did not comply with the requirements of 1926.550(a)(15) when working or being moved in the vicinity of power lines or energized transmitters: a) On or about October 23, 2012, while working on Virginia Street in Brighton IL, the Kobelco excavator made contact with 220 volt electrical overhead power lines which produced a spark the ignited natural gas. The excavator was not kept at a distance from the power line of the minimum clearance distance of 10 feet.

29 CFR 1926.652(a)(1): Each employee in an excavation was not protected from cave-ins by an adequate protective system designed in accordance with 29 CFR 1926.652(c): a) Jobsite on George Street, Brighton Il, at an excavation 9 feet deep, with no sloping of the trench walls, and the trench box was not installed, employees had accessed the trench to locate the end of the sewer lines.
