On or about 11/27/2012 at 200 McIntyre Drive, Hartwell, GA: A forklift in need of repair was returned to service before the repairs were made. Employees were exposed to a struck-by hazard when the repairs were not made.

29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks.    On or about 11/27/2012 at 200 McIntyre Drive, Hartwell, GA: Employees were exposed to various machine-guarding hazards, to include, but not limited to amputation, caught-by, and struck-by hazards at the following locations:    a) In Prototype, Lathe, Cubic LS 1840G was missing a chip guard. Employees working at the lathes in the prototype area are exposed to a struck-by hazard from flying debris while turning parts.     b) In Prototype,  Lathe, Cubic LS 1860G was missing a chip guard. Employees working at the lathes in the prototype area are exposed to a struck-by hazard from flying debris while turning parts.    c) In Prototype, Lathe, Leblond 804029R was missing a chip guard. Employees working at the lathes in the prototype area are exposed to a struck-by hazard from flying debris while turning parts.    d) In Prototype, Lathe, Cubic LS 1840G was missing a chip guard, Employees working at the lathes in the prototype are are exposed to a struck-by hazard from flying debris while turning parts.    e) CVRTD  Boot Inverter, Department #22 - Employees working at the Boot Inverter were exposed to a caught-by hazard when the Plexiglas guard on the Boot Inverter was removed and never replaced.    f) Wash-Line, Department #15 Employees working on the Wash-Line putting parts into baskets were exposed to a caught-by hazard while walking throough the moving conveyor line of baskets.    g) Rancho Labeler, Department #18 -  Employees working at the Rancho Labeler were exposed to a caught-by hazard when the diamond guard on the left side of the Rancho Labeler did not completely cover the ingoing nip points.      h) Baseline Assembly Welder, Department #23, K-Line -  Employees working at the Base Assembly Welders were exposed to a caught-by hazard from the unguarded in-going nip point created by the knurls.    i)  Prototype, Horizontal Band Saw  Employees working with the horizontal band saw are exposed to a caught-by hazard when more than the operating section of the blade was not guarded. The employees used to band saw to cut angle, round and flat metal stock.     j) Base Assembly Welder, Department #15- Employees working on the Base Assembly Welders are exposed to a caught-by hazard created by an in-going nip point of the knurls.    k) Base Assembly Conveyor, Department #15- Employees working at the Base Assembly Conveyor, filling baskets are exposed to a struck-by hazard when they walk between the moving baskets.    l) Tube Forming Welding Machine, Department #12, Strut Tube Mill - Employees at the tube forming welding machine were exposed to a struck-by hazard from the rotating spools when the mill was operational.    m)  Strut Tube Mill, Department #12, Forming Area -  Employees working are exposed to a caught-by hazard when the gates guarding the tube mill are left open.    n) Tubing Department, Multi-press -  An employee working at the multi-press was exposed to a caught-by hazard from the automatic rotating table on the multi-press. The automatic rotating table needs to be guarded.    o) Tube Mill, Butt Welder - An employee making adjustments to two bolts on the bottom of a shear on the Butt Welder, was exposed to a caught-by hazard. On 1/10/2013 the Butt Welder was not guarded.    p) Bodine - An employee cleaning the crump head on the Bodine was exposed to a caught-by hazard, when he had to reach into the machine. There was an opening that the employee put his hand through to clean the crump head.    q) Thielenhaus Rod Finisher, Department #21 -  Employees at the Rod Finisher are exposed to a caught-by hazard when the guard covering the chain drive was removed and never replaced. Tenneco Automotive Operating Company, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard

29 CFR 1910.242(b): Compressed air used for cleaning purposes was not reduced to less than 30 p.s.i.      On or about 11/27/2012 in the Prototype Department at 200 MyIntyre Drive, Hartwell, GA: Employees working on lathes, milling machines and "column and knee" mills were exposed to the struck by hazard from flying metal debris. The compressed air used to clean parts and working surfaces was measured to be approximately 93 psi.          Tenneco Automotive Operating Company, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard, which was contained in OSHA inspection number 314209248, Citation 1, Item 10, 01/010 and was affirmed as a final order on 11/03/2011, with respect to a workplace located at 200 McIntyre Drive, Hartwell, Georgia.     The Tenneco Automotive Operating Company was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29CFR1910.242(b) which was contained in OSHA inspection number 314209248 citation number 01, item number 10 and was affirmed as a final order on (date), with respect to a workplace located at200 McINtyre Drive, Hartwell, GA.
