29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladders length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed: a) At 1811 West Beach Blvd Gulf Shores, Alabama: On or about December 13, 2012 and at times prior; the employer exposed his employees to fall hazards in that he failed to ensure a portable extension ladder used by employees to gain access to and from the roof extended at least three foot above the landing area exposing his employees to fall hazards of 7.9 feet to the deck below.
