29 CFR 1910.179(j)(3): Periodic inspection. Complete inspections of the crane shall be performed at intervals as generally defined in paragraph (j)(1)(ii)(b) of this section, depending upon its activity, severity of service, and environment, or as specifically indicated below. These inspections shall include the requirements of paragraph (j)(2) of this section and in addition, the following items. Any deficiencies such as listed shall be carefully examined and determination made as to whether they constitute a safety hazard: The employer failed to ensure that overhead gantry cranes throughout the facility were periodically inspected: 1. 29 CFR 1910.305(b)(2)(i): All pull boxes, junction boxes, and fittings shall be provided with covers identified for the purpose. If metal covers are used, they shall be grounded. In completed installations, each outlet box shall have a cover, faceplate, or fixture canopy. Covers of outlet boxes having holes through which flexible cord pendants pass shall be provided with bushings designed for the purpose or shall have smooth, well-rounded surfaces on which the cords may bear. a) Wash Room; An electrical box did not have a cover. The wires inside the box were exposed and live. b) Packing department: 1. Glue Machine; an electrical box on front of the machine was missing the cover. The box had tape over the wires inside which were live. 2. Ventilator machine; faceplate of the wall switch was missing. This condition exposed employees to an electric shock hazard.
