Section (5)(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to struck by hazards related to runaway all purpose containers. On or about December 13, 2012, at a U. S. Postal Service facility located at 17500 Oakwood Boulevard in Allen Park, MI, employees were exposed to crushing injuries due to the fact that All Purpose Containers (APCs) and baskets were not positively connected to Toyota electric towing tractors. Up to three APCs and baskets were being towed at a time within the facility by towing tractors that have disconnected causing runaway loads weighing 100-600 pounds per APC or basket totaling 1,800 pounds.

29 CFR 1910.22(b)(1): Where mechanical handling equipment was used, sufficient safe clearances were not allowed for aisles, at loading docks, through doorways and wherever turns or passage had to be made and aisles and passageways were not kept clear and in good repair, with no obstruction across or in aisles that could create a hazard: On or about December 13, 2012, the employer did not ensure that sufficient safe clearances were allowed for aisles where mechanical handling equipment, such as wire cages up to 44 inches wide being pulled by the towline convyers, was used. The width of the aisles was approximately 78.5 inches. The recommended aisle width is at least 3 feet wider than the largest equipment to be utilized.
