29 CFR 1926.451(f)(7): Scaffolds were not erected, moved, dismantled, or altered, by trained and experienced employees under the supervision and direction of a competent person qualified in scaffold erection, moving, dismantling or alteration.  On or about December 11, 2012, employees erected scaffolding to construct wood awnings at three entrances to the building by inexperienced and untrained employees and the erection of the scaffold was not supervised by a competent person, exposing employees to a fall hazard.

29 CFR 1926.454(a): The employer did not have each employee who performed work while on a scaffold trained by a person qualified in the subject matter to recognize the hazards associated with the type of scaffold being used and to understand the procedures to control or minimize those hazards.  On or about December 11, 2012, employees were constructing wood awnings at three entrances to the building on a tubular welded scaffold without the employer having each employee trained, exposing employees to a fall hazard. The training shall include the following areas, as applicable:  a)  Any electrical hazards, fall hazards and falling object hazards in the work area;  b)  The correct procedures for dealing with electrical hazards and for erecting, maintaining, and disassembling the fall protection systems and falling object protection systems being used;  c)  The proper use of the scaffold, and the proper handling of materials on the scaffold;  d)  The maximum intended load and load-carrying capacities of the scaffolds used; and  e)  Any other pertinent requirements of this subpart.
